He was predeceased by: his parents, Guy Choate and Eales Choate (Baker); and his sister Sally Davenport. He is survived by: his wife Sue Choate (Green); his children, Guy Choate III (Debbie), Steven Choate (Sandra) and Cyndi Thomasson (Ralph); his stepchildren, Jim Fleming, Connie Semon, Becky McCleery and Jay Semon (Amy); and his niece April Hannant. He is also survived by 14 grandchildren, 13 great-grandchildren.
In lieu of flowers, memorial donations may be made in Guy's memory to the Flint-Belle Rive Cemetery Association and will be accepted at the service, or at Gholson Funeral Home in McLeansboro, IL.
