She is survived by: her husband Ralph R. Forys; her children, Richard Przada (Gail) and Rosemarie Sintzel; her grandchildren, Christine Mattson (Daniel), John Sintzel, Matthew Sintzel, Adam Sintzel, Michael Sintzel (Erin Smallmon) and Sharon Serafini (Dan); and her great grandchildren, Katherine, Daniel Mattson, Brooke, Charlotte Sintzel, Lauren and Logan Serafini. She is also survived by nieces, nephews, and cousins.
