He was predeceased by: his parents, Arthur Cook and Erna Cook (Bauer); his son Matthew T. Cook; and his grandson Nicholas Cook. He is survived by: his daughters, Dawn L. Cook of Belleville, IL, Kimberly K. Cook (David Starich) of Belleville, IL, Connie K. Cook of St. Louis, MO and Molly A. Cook of Glendale, UT; his daughter-in-law Laura Loehr of Belleville, IL; his granddaughters, Madeline Graham, Josephine "Josie" Graham, Devin Cook, Katy Graham and Lauren Ferrera (Joe); his great grandchildren, Aiden and Anthony Ferrera; and his sister Janet Schloemann (Roger) of Swansea, IL. He is also survived by many nieces, nephews and cousins.
