He is survived by: his children, Sam Pulliam, Jr. (Rachel) and Machelle Holt (Brian); his grandchildren, Chad Holt (Amber), Amy Holt, Gary Douglas, Dustin Pulliam (Arianna), Tara Heath (Brandon), Jamie Pulliam (Susie) and Krystal Pulliam (Kim Sizemore); his great grandchildren, Kaylie, Zoey, Jocelyn, Kennedy, Braelynn, Knox, Madilynn, Layla and Nash; his sisters, Linda Nelson (Alan), Debbie Bentley (Woody) and Sandy Zieba (Chet); and his caregivers, Cathy and Brenda. He is also survived by many nieces and nephews.
