He was predeceased by: his parents, Theophil "Theo" Czok and Agnes Czok; and his siblings-in-law, Donald Lutes and Martha Rogers. He is survived by: his wife Michelle Czok; his sister-in-law Margaret Lutes of Coal City, IL; his niece Morgan Rogers of Braidwood, IL; and his close friend Roger Gonis of Braidwood, IL. He is also survived by numerous nieces and nephews.
