He is survived by: his children, Tracy Ford of Canton, Christine Lindsey (Kirk) of Canton, Jeff Slayback (Cheryl) of Carlinville and Brian Slayback (Joce) of Lewistown; his siblings, Henny Deaton and Tommy Slayback; his grandchildren, Celesta England (Brian), Kara Simpson (Chris), Steven Slayback (Melissa), Sarah Slayback (Jacob Burton), Jenny Gabriel (Jared), Jeremy Hall (Beth), Anne-Marie Tucker (Jay Phillips), Aimee Jo Vanmiddlesworth (Brandon), Jacob Tucker, Amber Lyn Tucker (Devin Maggio), Nicholas Slayback, Hannah Buhl (Jeff), Vanessa Macke (Tyler) and Anthony Slayback II; and his children-in-law, Lyn Slayback and Billy Herald.
