He was predeceased by: his parents, Joseph "Tony" Costello and Wanita "Sally" Costello; and his brothers, Tom Still and Donald "Arkie" Still. He is survived by: his wife Debra Costello (Welte); his sons, Benjamin and Ian Costello; and his siblings, Mary Ann Wilson (Charlie), Ruthie Lang (Don), Carol Jo Costello-Jose, David "Buck" Costello (Glenda), Lisa Ayres (Bob) and Sara Klaus (Jon). He is also survived by many nieces and nephews.

Memorials may be made to the family to help defer medical expenses.
