He was predeceased by : his parents, Lewis Cook and Frances Cook (Branberg); and his siblings, Bill, Don, Ray and Lois.

He is survived by : his wife Kathy (Donovan); his son Jim (Sara) of Geneseo, IL; his sisters, Mary Johnson of East Moline, IL and Rose Neppl of Peoria, IL; and his granddaughters, Taylor (Jake) of Colona, IL and Ava of Geneseo, IL. He is also survived by numerous nieces and nephews.
