He was predeceased by: his parents, Jacqueline Johnson and Thomas Johnson. He is survived by: his wife Michele Greta Galvin; his daughters, Kerri Crystal, Michaela Mae and Erin Lee; his siblings, Paul Johnson (Marion) and Lori Cook (Peter); and his nephews, Ben Cook, Josh Cook and Ryan Johnson.
