He was predeceased by: his daughter Carrie Lynn Kenney (Curtis); his parents, Tom Smith and Doris King Smith; and his brothers, Tom Smith (Irene) and Bruce Smith (Chris).

He is survived by: his wife Pamela Peterson Smith (Western University) of Illinois; his sons, Dave Jr. and Michael (Heather Oberrieder); his granddaughters, Ella and Ava; and his siblings, Richard and Kathy.

In lieu of flowers, memorial donations are suggested to a charity of the donar's choice or plant a tree in his memory.
