He was predeceased by: his parents, Floyd White and Ada May White (Jones); his wife Jane Ellen (Kiser); his son Robert F. Smith; his sons-in-law, Walter Siddall and Doug Shaw; and his nephew Tim Raycraft.

He is survived by: his children, David White (Ely), Danny White, Cheryl Shaw, Charles Smith (Bonnie), Susan Siddall and Lauri Cunningham (Eric); his grandchildren, Troy White, Zachary Weddle (Karyn), Chelsey Royle (Scottie), Thomas Shaw, Evan Cunningham and Elias Smith; his great grandchildren, Jackson, Decklan Weddle and Bellamy Royle; his sister Irma Raycraft; his nephew Todd Raycraft; and his little buddy Rabble.

In lieu of flowers, Jerry would like to have donations go to the Shriner's Hospital.
