She was predeceased by: her parents, William Gaffney and Opal Gaffney; her spouse Oden E Mattingly; her daughter Tina; and her brother Ronald Gaffney.

She is survived by: her children, Wilbur Mattingly (Kim), Cindy Warren (Rick), Robert Weller and Kathy Brewer; her niece Brenda Davis; her nephews, Ronald Gaffney and Kyle Gaffney; and her close friend Cindy Varner. She is also survived by 7 grandchildren, 3 step grandchildren, 1 great grandchild and 6 step great grandchildren.

Memorial donations may be made in Phyllis' name to the Vermilion County Animal Shelter.
