He was predeceased by: his parents, Lois Hayes and Richard Hayes; and his sister Ann. He is survived by: his friend Shirley Kistler; his sisters-in-law, Patricia Ryan and Terri Hall; his wife Pam; his daughters, Amy Hayes (Heather Moles) of Santa Barbara, CA and Angie Cook of Decatur; his grandchildren, Xavier Cook, Myah Cook, Avery Cook and Atton Hayes; his sister Patricia French of Lincoln, IL; Vern. He is also survived by many nieces and nephews.
