He was predeceased by: his parents, Frank J. II and Helen E. Andrlik II (Kysilka); his brother Allen Andrlik; his sister-in-law Margaret Simpson Baker; his uncle Erwin Pesek; and his aunt Blanche Kysilka Pesek.
He is survived by: his wife Patricia Ann (Simpson); his sisters-in-law, Andrea Andrlik, Carol Tetzloff (Robert) and Katherine Simpson (Robert Johnson); and his cousin Robert Andrlik (Sharon). He is also survived by two nephews and one niece.
