He was predeceased by: his parents, Kenny and Dee; his sister Elizabeth Yeo; his brother-in-law Bill Hurley; and his mother-in-law Elaine Hurley.

He is survived by: his wife Julie Yeo; his children, Elizabeth, Peter and Patrick Yeo; his siblings, Nancy Reazin (Troy) and Burt Yeo (Christy); his siblings' children, Mike, Pat, Danny Reazin, Kenny, Aaron, Adam Yeo, Libby Bolner and Hope Yeo; and his father-in-law Jerry Hurley.

In lieu of flowers, donations to one of Jack's favorite charities would be greatly appreciated: Homewood Cancer Support Center (https://www.cancersupportcenter.org) or Chicago White Sox Charities - Youth Baseball.
