He was predeceased by : his parents, Leslie Dale and Gladys Gwendolyn Mayo (Liston); and his brother Jerry D. Mayo. He is survived by : his former life partner Nancy M. Kolb; his stepchildren, Kris Mayo (Warrenville), Kurt Mayo (Heidi McMurren) and Kory Jackson (Germantown Hills); his wife Cynthia M. Fitzpatrick Smith; his stepdaughters, Tricia Mason (Dunlap), Jeni Sander (Peoria) and Michele Smith (Mapleton); his granddaughters, Lindy and Layne Jackson; his stepgrandchildren, Logan, Avery Mason, Dakota Smith, Chloe Sander, Blake, Kaitlyn and Jamie Vest; his sister Vicki Mayo (Sterling Kernek); and his sister-in-law Patricia Mayo (Plymouth). He is also survived by three nieces and one nephew.

Memorial contributions may be made to the ALS Association.
