He was predeceased by: his father David Hoffmann. He is survived by: his mother Susan Hoffmann (Fuqua) of Porum, Oklahoma; his children, Daniel Hoffmann of Earlville, Illinois and Annistyn Hoffmann of Earlville, Illinois; his brother Tavis Hoffmann (Cristi Cain) of Porum, Oklahoma; his goddaughter Taya Hoffmann of DeKalb, Illinois; his stepson Levi Alexander (Grace Keller) of Somonauk, Illinois; and the mother of his children Angela Henczel. He is also survived by many cousins, step-nieces, step-nephews.

In lieu of flowers, and to honor Dan's memory, the family asks that donations be made to benefit the care and education of his surviving children.
