He was predeceased by: his parents, Alfred Cunningham and Anna Cunningham (Anderson); and his siblings, Patricia Salyers and Allan Cunningham. He is survived by: his children, Susan Frejofsky (Gary) of Aurora, OH and Mike Cunningham (Holly) of Effingham, IL; his grandchildren, Dylan, Abby, Riley and Sydney; David Cunningham (Patricia) of Oklahoma City, OK..

Memorials in Herb's honor may be made to the Effingham Sportsbackers and will be accepted by McCracken-Dean Funeral Home who is assisting the family.
