He was predeceased by: his parents, Andrew Polarek and Josephine Agnes Polarek (Komasa); his wife Catherine Polarek; his brother Melvin Polarek; his sons-in-law, Wes Anderson and Brad McDonald; and his great grandchild Silas Jaxon Burdette.
He is survived by: his children, Lisa Anderson (Mike Roche) of Manhattan, IL, Laura Didier of Effingham, Ken Polarek (Theresa) of Marco Island, FL, Mary Beth McDonald of McDonough, GA and Michelle Hieserman (Bryan) of Atlanta, GA; his grandchildren, Ryan (Emily), Nicholas (Amy), Jaclynn, Justin (Nicole), Megan, Abigail (Jesse), Torrie Clare, Tara Rose (Cameron), Ali, Elm (Chris), Meghan (Grant), Kenny (Dani) and Colin (Kathleen); his great grandchildren, Sydney, Scout, Lily, Natalie, Chloe, Jayden, Marley, Aaro, Olivia, Violet, Thomas and Jameson; his brother David Lee Polarek; and his siblings-in-law, William Ormrod (Debbie) and Sandy Polarek. He is also survived by nieces and nephews.
In lieu of flowers and gifts, memorials may be made to St. Anthony Church or St. Anthony Schools.
