He is survived by: his wife Barbara; his children, Jessica Lambert (Tim), Dan Cusack (Kate Neuens), Matt Cusack and Nate Lambert; and his siblings, Martin (Patti), Maryann and Jeanmarie Cusack.
Visitation will be held on Friday, October 23rd 2026 from 9:30 AM to 11:30 AM at the St. John Fisher Catholic Church (10300 S Fairfield Ave, Chicago, IL 60655). A funeral Mass will be held on Friday, October 23rd 2026 at 11:30 AM at the same location.
