He was predeceased by: his wife Helen E.; and his siblings, Emily Biernat (Walter), Chester Polak (Dorothy), Mary Slowiak (Vince) and Edward Polak (Dolores).
He is survived by: his children, Charlene Piech (Joseph), Anthony (Mary) and David Polak (David Beran); his grandchildren, Jamie Mateicka (James) and Taylor Ehlers (Karl); his great grandchildren, Aubrey and Alexandria; and his sisters, Rose Sromek (late Stanley) and Harriet Milstroh (late Richard). He is also survived by many nieces and nephews.
