She was predeceased by: her parents, Joseph LaMontana and Florence LaMontana (Greco). She is survived by: her children, Richard Dvorak Jr. (Carolyn), Paul Dvorak (Heather), Jennifer Forsythe (Jason) and Anthony Dvorak (Sara); her grandchildren, Richie, Mary and Abby Dvorak; her children, Emily, Christian, Austin Dvorak, Reese, Lucas Forsythe, Koren, Francesca, Nicolette and Jackson Dvorak; and her brother Dominic LaMontana. She is also survived by nieces and nephews.
