He was predeceased by: his parents, Donald Hazelwood and Margie Hazelwood (Maxfield). He is survived by: his wife Arlynn Hazelwood (Ashlock); his children, Sharon Ward (Tim), Britt Hazelwood, Cheryl Morris (Jeff) and Becky Middendorf (Steve); his grandchildren, Lynna Rippy (Aaron), Kale Ward, Cheyenne Hazelwood, Alyssa Vetere (Eric), Drake Morris, Jacob Middendorf (Hollyann), Jeremy Middendorf (Katie) and Grant Middendorf; and his sister Lucy Reznicek. He is also survived by 7 great grandchildren, many cousins, nieces, nephews and friends.
