He was predeceased by: his father Robert Ax; his stepfather Mike McLean; his sister Errin Ax; his aunt Sharon Kinkead; and his uncle Jim Scaturro.
He is survived by: his mother Charolette McLean; his siblings, Kim Castelli (Gary), Matt and Chris Ax; his children, Carley Dortmann and Tylar Cotton; his grandson Kylan Anthony; his close friend Angie (family: Dave); his nieces and nephews, Caitlyn Gourley (John), Nikki Procasky (Nick), Allix Jakich, Dominic Jakich and Josh Castelli (Danielle); and his cousin Christy Kostoff. He is also survived by great-nieces and great-nephews; a host of beloved Aunts, Uncles.
