Jack D. Smith Obituary

In loving memory of Jack D. Smith of Havana, Illinois, who departed this world on September 8, 2025 at the age of 85. Jack leaves behind a legacy remembered by family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He was predeceased by: his parents, Otis O. Smith and Orlana M Smith (Williams); and his siblings, Dawnie List, Roy Richard Smith, Lois Giebelhausen and Sally Jackson.

He is survived by: his son Roy Anthony "Tony" Smith of Peoria; his stepchildren, Wayne Wallace of Moncks Corner, Dave Wallace of Bartonville, Illinois and Renee Shannon of Battle Ground, Washington.; and his sister Alta Hackett (Jess) of East Peoria, Illinois. He is also survived by thirteen nieces and nephews; and many great nieces and nephews.

To honor his memory and in lieu of flowers, the family asks that donations be made to Toys for Tots.
