He was predeceased by: his wife Carolyn A. Kozlowski; his parents, Witol Kozlowski and Phyllis Kozlowski; and his siblings, Florence (late Edwin), Carl (late Gladys), Joseph, Dolores Elwood (late Bill), John Jr. Kolinski (late Ruth), Estelle Meehan (late Edward), Theresa Kolinski and Jim Kolinski (late Ella Marie).
He is survived by: his daughter Denise Duffing (Jesse); and his granddaughter Isabella Rose Duffing. He is also survived by cousins and friends.
