Hajek (Pat); and her grandchildren, Theresa Cooper, Lynda Cooper-Boyke (Ryan), Jessica Adamov (Petar), Bill Melka (Nicole), Cody Melka, Charles Hajek (Beverlee), James Hajek (Katy), Daniel Hajek and Joseph Hajek (Tatiana). She is also survived by her grandchildren and great grandchildren.
