He was predeceased by: his parents, Donald Puyear and Fern A. Puyear; his siblings, Bill Puyear and Janet Scheurich; and his nephew Larry Puyear.
He is survived by: his significant other Donna Lutkehaus; his son Cory Puyear (Jean); his grandsons, Joey and Josh Puyear; his sister-in-law Cleo Puyear; and his nephews, Jim Puyear (Cyndi) and Dale Puyear (Shirley).
In lieu of flowers, memorials may be made to Illinois State University Youth Football Summer Camp or St. Jude Children's Research Hospital.
