He was predeceased by: his son Caleb Dean Coultas. He is survived by: his parents, Carl Coultas and Ruthie Mason Coultas of Jacksonville; his wife Nikki Lynn Leffler; his children, Joshua Coultas (Regina Fry) of Jacksonville and Danielle Coultas (Cody Walker) of Jacksonville; his stepchildren, Nicholas Lockhart of Hawaii and Alyssa Lockhart of Jacksonville; and his siblings, Brian Coultas (Amy) of Jacksonville and Kelly Martin (Scott) of Jacksonville. He is also survived by 12 grandchildren, several nieces and nephews.
