He was predeceased by: his parents, Carl S. Hawkins and Jaunita Hawkins; and his brother James W. Hawkins. He is survived by: his wife Patricia A. Harmon; his sons, Carl J. Hawkins (Sherry) and David W. Hawkins (Vicki); his grandchildren, Jessica, Alexis, Hailee and Olivia; his great grandchild Huxley; and his sister Carolyn Perry (Drexel).
In lieu of flowers, memorials may be made to Lightways Hospice or RUSH Cancer Center.
