She was predeceased by: her husband John "Jack" Francis Bolek; her siblings, Eugene Kuzma (Joanne), Jim Kuzma and Carol Allen (Kuzma); her brothers-in-law, Dan Allen, Jim Holbrook and Eldon Fraser; her grandson Nicholas Bolek; and her parents, Frank Kuzma and Agnes Kuzma.
She is survived by: her children, Janet Rotz (Michael) of Downers Grove, Joni Bolek of Joliet, John Bolek (Tiffany) of Plainfield and James Bolek (Traci) of Sylvania, OH; her grandchildren, Dana, Katie Richmond (Jack), Emma Rotz, Jack, Alex, Joey Casado, Josh, Jenna, Emily Bolek, Kristin Leschinsky (Nic), Hannah Risner (Braden), Andrew, Mathew and James Bolek (Nelly); her great grandchildren, Carter Richmond, Peyton, Maya Risner and Janel Bolek; her sister Barbara Holbrook-Fraser; and her brother-in-law Robert Bolek (Marie). She is also survived by many cousins, nieces and nephews.
