He was predeceased by: his parents, Nathaniel Ford and Carrie Brooks; his wife Juanita; his children, Nathaniel, Jeanette and Veronica; his great grandchild Kaden; his brothers, Joe Mack (Kankakee), Willie Mack, Tommy Brooks and Roosevelt Brooks; his nephew Jerry Johnson; and his son-in-law Howard Perkins.
He is survived by: his son Sidney Shaw (Chevon); his granddaughters, Annette Ford-Williams (Philander), Twanda Perkins, Vonita Perkins (Kevin) and Cara (Carderious Perkins-Day); his siblings, Margaret Brooks (Mich), Nancy (Ray), Shelby (MS), Felita'Tess'Brooks (MS) and Albert (Florence); his close friends, Beverly Jones (Kankakee) and B. C. Harris; and his niece Jannie King. He is also survived by his eighteen great grandchildren, seven great-great grandchildren.
