She was predeceased by: her parents, Lee Collins and Martha Collins (Langfield); her husband George M. "Tony" Jack; and her children, Benjamin Jack and Grace Lornitis.
She is survived by: her sons, Myron Jack (Pam) of Decatur, IL and Franklin Jack (Kelly) of Kincaid, IL. She is also survived by several granddaughters, great grandchildren and great-great grandchildren.
