He was predeceased by : his parents, Thomas Joseph and Kathryn Cusack (Walsh); and his siblings, Mary Eileen Greer and Thomas Cusack. He is survived by : his wife Joyce Cusack (Yglesias); and his sisters, Kathryn Levenshus (Stuart) of Westminster, CA and Jane Mosier of Chicago, IL. He is also survived by nephews and Godchildren.
