He is survived by : his wife Deborah Jo; his mother Marcia (Houk); his sisters-in-law, Stacy Collins (David) and Shelley Wade (Mickey); his nieces, Abigail Collins and Hannah Cook; his nephew Josiah Cook; and his brother-in-law Christopher Cook.
