He was predeceased by: his parents, Paul Forker and Marion Forker (Frye); and his wife Elizabeth Forker (Ann). He is survived by: his children, John Forker (Kim) of Lindenhurst, IL and Sue Smith (Kevin) of Cicero, IN; his grandchildren, Brandon Forker (Cortney), Morgan Forker, Nick, Katie and Lizzy Smith; and his sister Sally Lee (Steve).
