Wojciak of Lockport, Illinois, born in Chicago, Illinois, who passed away on June 19, 2022, at the age of 89, leaving to mourn family and friends. He was predeceased by: his wife Barbara Wojciak (Nee Urbanek); his granddaughters, Jessica Wojciak and Sheri Wojciak; his parents, Bernard Wojciak and Helen Wojciak (Nee Dzielak); his siblings, James Wojciak (Rose Marie) and Dolores Roberts (late William); and his nephew David Wojciak.
He is survived by: his children, Michael Wojciak (Sandra), Cathy Kuczek (Kenneth), Jennifer Hernandez, Lawrence Wojciak (Kimberly) and Carl Wojciak (Cynthia); and his grandchildren, Zachary Kuczek, Dominique Hernandez, Gianna Orlando, Hannah, Ethan, Aidan Wojciak, Rebecca Wojciak, Luke and Crystal Wojciak.
In lieu of flowers, memorials to Lightways Hospice in Tom's name would be greatly appreciated.
