He is survived by: his wife Sharon Cook; his children, Paul Cook (Pam), Glen Cook (Renee), Rachel Bloomingdale (Devin) and Sarah Carr (Jon); his grandchildren, Stefan Cook, Kiersten Bloomingdale, Makayla Bloomingdale and Nathan Carr; and his siblings, Donna Cook and Joe Cook.
