He is survived by: his parents, Dean J. Cook and Phyllis R. Cook (Combs); his wife Elizabeth Anne Cooper; his sons, Clayton Cook (Sarah) of Good Hope and Dylan Cook (Jill) of Sciota; his grandchildren, Oliver David, Elise Renae and Carson John; his sisters, Claudia Winterrowd (Bruce) of Springfield and Anita Bradley (Steve) of Cantrall, Illinois; his nephews, Adam and Ben Winterrowd; and his niece Rachel Cox (Logan). He is also survived by many cousins and dear friends.
