He was predeceased by: his parents, Robert Hornback and Mary Hornback; his wife Rachelle Hornback; and his brother Paul Hornback. He is survived by: his children, Jeremy Hornback, Justin Hornback (Paige) and Jessica Hornback (Tyler); his grandchildren, Evan, Brodi, Kylee, Kaydence, Raelynn, Colsen Hornback and Bryer Duncan; and his brothers, Danny Hornback (Kris) and Bobby Hornback. He is also survived by several nieces and nephews.
