He was predeceased by: his wife Phyllis; his parents, Alvin Hook and Verna Hook; and his sister Wilma Korf. He is survived by: his children, John, Jeff (Julie), Suzanne (James) and Howard Jr. (JR); the mother of his children Betty; his stepdaughter Debbie; his grandchildren, Rachel (Andy), Darrin (Amy), Ashley and Ivan; his stepgrandchildren, Josh (Lauri), Jordan and Shianne; and his siblings, Ed (Kay) and Ellen (Rick). He is also survived by many extended family members and friends.
