He was predeceased by: his parents, Alfred Chouinard and Clara Chouinard (Barthel); and his brother Francis Chouinard. He is survived by: his wife Jeanette Chouinard; his children, Greg Chouinard (Karin), Jeff Chouinard (Mary Ann), Mark Chouinard, Scott Chouinard (Claire) and Shelley Chouinard; and his siblings, Elwin Chouinard (Betty), Mary Ann Perman (Terry), Diane Gillan (Alan), Sharon Johnson (Les), Michael Chouinard (Patty), Paul Chouinard and Donna Voght. He is also survived by his 6 grandchildren, and 2 great-grandchildren.
In lieu of flowers memorials may be made to St. Jude Children's Research Hospital.
