He is survived by: his wife Patricia Leuntjens; his sons, Jeffrey Wayne Culver (LuAnn) and Perry Carl Jacobs II; his grandchildren, Jacob, Zachary Culver and Lauren Jacobs; his brother Ken (Chris) of Bowlinggreen, KY; his nephews, Andy and Alex; his niece Julie; his sister-in-law Barbara Benson; and his nephew Justin.
