He was predeceased by: his parents, Kenneth Polark and Lucille Polark (Johns); and his brother Dennis. He is survived by: his wife Mary Jo; his children, Brian, Elizabeth and Michael (Amy); his grandchildren, Collins, Louise, Francis, Cora James and Theodore James; and his siblings, Roger (Bobbi Jo) and Pamela. He is also survived by many brothers- and sisters-in-law, nieces, nephews and dear friends.
Visitation will be held on Monday, October 12th 2026 from 3:00 PM to 8:00 PM and on Tuesday, October 13th 2026 from 10:00 AM to 11:00 AM. A funeral Mass will be held on Tuesday, October 13th 2026 at 11:00 AM at the St. Raymond de Penafort Church (301 S I Oka Ave, Mt Prospect, IL 60056).
In lieu of flowers, memorials may be made to St. Raymond Parish.
