He was predeceased by: his wife Rita Pellegrini; and his siblings, Vicki Beavers and Delbert Lee. He is survived by: his parents, Thomas Alvin and Sarah Juanita Cook (Jones); his sons, Garrett Cook (Sheri) and Loren Cook (Kim); his grandchildren, Brendon Cook (Meaghan), Ethan Cook (Madeline), Sydney Cook, Shelby Cook and Sarah Cook; and his nephews, Kevin Beavers (Denise), Ryan Beavers and Michael Mifflin.
In Lieu of flowers, the family requests with gratitude that memorial contributions be made to the Murphysboro Knights of Columbus.
