He is survived by: his wife Nancy M. Polivka; his children, James Bradford Polivka (Debbi), George Bryan Polivka (Jeri), Dwight Chandler Polivka (Jana) and Rebekka Polivka James (Duane); his grandchildren, Robert Tucker DeBolt, James Harrison Polivka, Caleb Preston (Rae), Jonathan Chandler, Joseph Bradford, Terry Anthony James, Rebekah Jeanne Schneider (Joshua) and Anna Ruth Hess (Victor); and his great grandchildren, Preston James Nathanael, Phinehas David Schneider and Boaz Dwight Schneider.
