He was predeceased by: his parents, Raymond Jensen and June M. Kozak; his brother Dean Kozak; and his stepfather Chester Kozak. He is survived by: his siblings, Linda Zachara (Ron), Gregory Kozak and Catherine Rucker (James). He is also survived by Nieces and Nephews.
