He is survived by: his mother Donna Swayze (Haake) of Nashville; his wife Cheryl Cook of DuBois, IL; his children, Taylor Cook of DuBois, Olivia Cook (Gavin Love) of DuBois and Carson Cook (Caitlynn Pytlinski) of DuBois; his siblings, Shelia Frieman (Bryan) of Ashley, Chris Olney, Heidi Morrison (Roger), Todd Linnell and Ed Linnell (Huyen); his parents-in-law, Warren and Shirley Steinkamp of Addieville; his brother-in-law Mark Steinkamp of Addieville; his nieces and nephews, Kailie Frieman (Greg Powers) of Ashley, Coty Frieman (Erin) of Ashley and Makenzie Steinkamp of Nashville; his great-nieces and great-nephews, Brynner Inman, Huxlie Reel, Aayden Gerstner, Kehnley Frieman and Archer Allen; and his godparents, Alita Schorfheide and Glenn Haake. He is also survived by other relatives and a host friends.
Should friends desire, memorials may be made to the ALS Association, Jared Burke Foundation, or St. Paul United Church of Christ and will be accepted by the Campagna Funeral Home in Nashville, IL, who is assisting the family with arrangements.
