He was predeceased by: his parents, Julia Bartley and Ralph Bartley; and his siblings, Barbara, Ralph Jr., Charles and Frances.

He is survived by: his wife Faye; his children, Erin Cummisford (Rick) and Tim (Christi Smith); and his grandsons, Ian, Brent and Victor.

In lieu of flowers, contributions to the VITA Foundation are welcome.
