She was predeceased by: her husbands, John Surma and Edward Petrasek; her children, Debra Horner, Mark Petrasek and Joseph II; and her parents, Peter Rzeszut and Catherine Rzeszut.
She is survived by: her children, Paul Petrasek, Elizabeth Nazorek (Mark), Laura Slak, Robert Petrasek Sr., Donna Black, Allison, John, Kristen, Crystle, Gregory, Chanel, Edward, Taylor, Jaclyn and Jonathan; her great grandchildren, Colton, Maya, Mason, Aspen, Joseph III, Vincent, Jacob, Leo and Brianna; Anna Rzeszut, Sophie Kozak, Cecelia Waszak, Irene Nykiel and Theresa Mierzwa.
In lieu of flowers, donation to the American Cancer Society would be appreciated in c/o Genevieve "Jean" Surma Petrasek.
